since previous change relating to incomplete metafile's being read while
still being written, there are still circumstances that result in failure
This change tracks last_good_pos after each full section and advances
mf->pos once by parsed buffer prefix so the next read does not skip tail
bytes. The log lines have now been made dbg instead of warnings as they
should be handled correctly on subsequent reads of the file

when under load it's possible that a metafile gets updated by rtpengine
while it's still being read by the recording daemon. this can result
in a WARNING being logged and the loop breaking. That section ends up
unprocessed and any changes in it don't get applied.
This change only updates the offset after a successful section read.
Therefore, if a section is incomplete and the loop breaks, the next
inotify event will retry that incomplete section and it should succeed

The mix sink points into the tls_fwd object, so we must not free it
during operation, such as when a connection error occurs.
Separate out object creation and connection setup functions. Reset
object state at connection error, but don't free the object.
Make reconnect attempt part of checking the connection state.

Delay writing of call DB entry until a metadata has been set, even if
empty. This is so that we can know whether "skip DB" was set or not.
Check DB entry status with every event, not just when streams are
created. Stream may get created before all relevant data for DB entries
is there.

Move destination format information into the sink object.
Decode each codec directly to its matching output format and leave
resampling to the sinks.
Move managing of the adjusted multi-channel output format into the mix
context.
Make sure all inputs to a mix outputs use the same audio format.

The GLib slice allocator has been obsoleted. Having a mixture of two
allocators can lead to hidden issues on systems that use different
implementations. Take the leap and replace everything with g_new/g_free.

mixed audio file
When a mixed wav file is created, the channels in the wav container are
currently allocated in the same order as each SSRC is received, meaning
it is impossible to know which channels have been allocated to the offer
or answer side of the call. Furthermore if there is a reinvite or media
file played, these are also allocated in the order that SSRC is received
- so an "answer" could end up sharing a channel with an "offer" with no
way of knowing this.
This patch allows you to specify how many channel slots should be
allocated within the mixer, and allows you to then specify which slot is
assigned to each media in the call (this will usually be 2 slots in
total, slot 1 for answer, slot 2 for offer or vice versa).

Handle the `output-destination` flag in the same way as the `metadata`
flag, eliminating the need to do it explicitly everywhere it's relevant.
Add a few alternative spellings of `output-destination`.
Rename the member from flags to `recording_file` to make it more clear
what it is.
Add save/restore capabilities of that field to the Redis code.
Unify printing of monologue LABEL into the same function that handles
the metadata.
Update documentation to better explain this option, and use the new
clearer name.

Packet processing (decoding, encoding, file writing, etc) happens with
the main parent object (metafile) unlocked and only with the specific
child object (SSRC) locked.
During teardown we close all SSRC objects with the metafile locked,
without locking the SSRC objects. This opens up a race condition during
which SSRC closing can happen at the same time as packet processing.
Fix this by locking each SSRC object during teardown, and also clear out
the SSRC hash table while the metafile is locked, which serves as a
marker to a packet processing thread that processing should be skipped.

When the `discard-recording` flag is given in one of the commands to
rtpengine (e.g. in the `delete` command), the metafile is renamed to a
.DISCARD suffix and then deleted.
The recording daemon then, seeing the .DISCARD suffix, proceeds to
immediately close all recordings, delete the files if any, and delete
the entries from the DB.

This is the "type" of the recording, but sadly we already have "type"
with a slightly different meaning (i.e. the SSRC in hex for single
streams), so we name this the recording "kind".

Instead of waiting for the garbage collector to clean up the SSRCs
objects, do it immediately when a delete is seen. This closes output
files and TLS connections more quickly.

This prevents empty mixed output files from being created when mixed
output is enabled in the config but recording isn't active for that
call.

Keep track of how many streams have been added to the DB for each call.
Then at the end of the call, if there are no corresponding streams in
the DB, delete the entry for the call instead of marking it as complete.
This eliminates DB entries for calls that were cancelled or failed.
