The GLib slice allocator has been obsoleted. Having a mixture of two
allocators can lead to hidden issues on systems that use different
implementations. Take the leap and replace everything with g_new/g_free.

Grab source monologue and source media only once, and pass the object
down to the various functions, instead of doing the retrieval several
times in multiple places.

If the receiver of a previously passthrough T.30 stream gets switched to
a generated PCM stream from a T.38 gateway, continue the sequencing of
the previous SSRC. Technically this is not necessary as the generated
PCM stream gets a new SSRC, but at least Asterisk seems to ignore this
and expect sequencing to continue, and will ignore PCM if the sequence
jump is too large.

Make sure a codec is not only known to us, but that it can actually be
used, in places where it makes sense. This is partially redundant
because ensure_codec_def_type already takes care of this, but a codec
definition may come from a different source, so it doesn't help to
double check.

Make the macro return the appropriate pointer type, and make sure the
free function takes an argument of the same type. This also eliminates
some boilerplate type-casting code.

Make sure sdp_attributes is only used for _received_ SDP attributes, and
not ones that we want to send out. The only places where self-generated
attributes are being used is the T.38 gateway.

Some source strings are `const char *` and we don't have an alternative
`str` type pointing to a const buffer. Strictly speaking some buffers
aren't even char buffers, yet we use the `str` type to manage them. This
eliminates the need to explicitly cast every usage.

In the file implementations follow the rules:
1. Firstly goes the correlated header file, then one empty row.
2. Secondly go system headers, so in angle-brackets, then one empty row.
3. Thirdly, go custom header files, so in double quotes,
then one empty row.
4. If there is "xt_RTPENGINE.h", it's mentioned next, but separately,
then one empty row.
5. If there are pre-processor definitions, they are added.
6. And eventually at least one empty row before the code.
In some situations it's allowed to step aside from the rules,
when inclusions are dependent on each other, so on the sequence,
and also possibly on some inline objects definitions, but if possible
to follow the rules, it's being done.

Split out the logic to determine the destination payload type into a

*) split codec manipulation (strip/offer/accept/etc) into separate
functions for clarity and better code maintenance
*) fully update codec handlers in both directions after an answer
*) explicit allocation and handling of codecs and payload types in a
codec_store object
*) improve codec matchup logic during answer
*) more explicit handling of supplemental codecs (CN/DTMF)
*) remove now obsolete hacks for handling certain use cases

This makes the type in line with string(3) functions and eliminates some
