MT#58547 voip_peer_host change relation to lcr_gw

* relation to lcr_gw is now by foreign.id instead of foreign.flags,
  that is to reflect the change that provisioning.voip_peer_hosts.id
  and kamailio.lcr_gw.id is now equal and lcr_gw.flags no longer holds
  the voip_peer_hosts id reference
