When in single-argument mode (very rarely used), a malformation of a column
name (also very rare) could cause a NULL to be returned when retrieving the
channel variable for that column. Passing that to strncat causes a SEGV. We
now check for the NULL and print a warning message.
Resolves: #1101
* Do a git blame on the embedded XML application or function element.
* From the commit hash, grab the summary line.
* Do a git log --grep <summary> to find the cherry-pick commits in all
branches that match.
* Do a git patch-id to ensure the commits are all related and didn't get
a false match on the summary.
* Do a git tag --contains <commit> to find the tags that contain each
commit.
* Weed out all tags not ..0.
* Sort and discard any .0.0 and following tags where the commit
appeared in an earlier branch.
* The result is a single tag for each branch where the application or function
was defined.
The applications and functions defined in the following files were done by
hand because the XML was extracted from the C source file relatively recently.

This patch makes it so only matching non-empty key names, and keys created by
the HASH function are eligible for inclusion in the comma separated string. It
also fixes a bug where it was possible to write to a negative index if the
result buffer was empty.

Feeding LISTFILTER an empty variable results in an invalid ERROR message.
Earlier changes made the message useless because we can no longer tell if
the variable is empty or does not exist. It is valid to try to remove a
value from an empty list just as it is valid to try to remove a value that
is not in a non-empty list.
* Removed the outdated ERROR message.
* Added more test cases to the LISTFILTER unit test.

ASTERISK_REGISTER_FILE no longer has any purpose so this commit removes
all traces of it.
Previously exported symbols removed:

This also removes the mtx_prof static variable that was declared when
MTX_PROFILE was enabled. This variable was only used in lock.c so it
is now initialized in that file only.

Git does not support the ability to replace a token with a version
string during check-in. While it does have support for replacing a
token on clone, this is somewhat sub-optimal: the token is replaced
with the object hash, which is not particularly easy for human
consumption. What's more, in practice, the source file version was often
not terribly useful. Generally, when triaging bugs, the overall version
of Asterisk is far more useful than an individual SVN version of a file. As a
result, this patch removes Asterisk's support for showing source file
versions.
Specifically, it does the following:
* Rename ASTERISK_FILE_VERSION macro to ASTERISK_REGISTER_FILE, and
remove passing the version in with the macro. Other facilities
than 'core show file version' make use of the file names, such as
setting a debug level only on a specific file. As such, the act of
registering source files with the Asterisk core still has use. The
macro rename now reflects the new macro purpose.
* main/asterisk:
- Refactor the file_version structure to reflect that it no longer
tracks a version field.
- Remove the "core show file version" CLI command. Without the file
version, it is no longer useful.
- Remove the ast_file_version_find function. The file version is no
longer tracked.
- Rename ast_register_file_version/ast_unregister_file_version to
ast_register_file/ast_unregister_file, respectively.
* main/manager: Remove value from the Version key of the ModuleCheck
Action. The actual key itself has not been removed, as doing so would
absolutely constitute a backwards incompatible change. However, since
the file version is no longer tracked, there is no need to attempt to
include it in the Version key.
* UPGRADE: Add notes for:
- Modification to the ModuleCheck AMI Action
- Removal of the "core show file version" CLI command

It is not apparent to the average user that the PASSTHRU function should not
be passed as ${PASSTHRU(string)} but just as PASSTHRU(string) to functions
which take a variable name and not its contents.
This patch clarifies the behavior in the documentation and provides an example.

Fix crash with STRREPLACE function.
The ast_func_read() function calls the .read2 callback with the len
parameter set to zero indicating no size restrictions on the supplied
ast_str buffer. The value was used to dimension a local starts[] array
with the array subsequently used.
* Reworked the strreplace() function to perform the string replacement in
a straight forward manner. Eliminated the need for the starts[] array.

* Add REPLACE function, which searches a given variable for a set of
characters and replaces each with a given character.
* Add PASSTHRU function, which passes a literal string back, like a NoOp for
functions. Intent is to be able to specify a literal string to another
function that takes a variable name as an argument.
* Let the array manipulation functions work with dialplan functions, in
addition to variables. This allows the array manipulation functions to
modify ASTDB and ODBC backends, assuming the func_odbc configuration has
both read and write functions.

Force an error if a blank is passed to QUOTE (because the documentation states the argument is not optional).
This change makes URIENCODE and QUOTE behave similarly, since the documentation
states that the argument is not optional, for both.

This patch adds 'const' tags to a number of Asterisk APIs where they are appropriate (where the API already demanded that the function argument not be modified, but the compiler was not informed of that fact). The list includes:
- CLI command handlers
- CLI command handler arguments
- AGI command handlers
- AGI command handler arguments
- Dialplan application handler arguments
- Speech engine API function arguments
In addition, various file-scope and function-scope constant arrays got 'const' and/or 'static' qualifiers where they were missing.

This branch adds additional methods to dialplan functions, whereby the result
buffers are now dynamic buffers, which can be expanded to the size of any
result. No longer are variable substitutions limited to 4095 bytes of data.
In addition, the common case of needing buffers much smaller than that will
enable substitution to only take up the amount of memory actually needed.
The existing variable substitution routines are still available, but users
of those API calls should transition to using the dynamic-buffer APIs.
